Use this form to notify the Office of the Vice President, Academic Affairs (VPAA) that an Academic Integrity Code violation occurred. The student will be notified that an Academic Integrity Code Violation Report has been filed and the student will be provided instruction for the academic appeal process. Please make sure to follow the below timeline. The below steps need to happen within two weeks of the violation:
1. Faculty must meet with student to discuss the violations.
2. If it is determined an academic integrity violation occurred, the faculty will determine a sanction stated in Regulation 6.3000, IV, D, 1, b and notify the student. (link to regulation above)
3. Fill this form out in its entirety which will be sent to the VPAA's Office. Reporters are responsible for sending a copy of this submission to the student they are reporting for an academic integrity violation.
4. If the student files an appeal and a hearing is granted, you will be contacted by the Office of the VPAA to set a date/time for the hearing.
